The primary mission of the Meadowood Health Partnership (MPH) is to strengthen families, promote literacy

and provide neighborhood-based model health care. The partners include, Meadowridge Library, Today

Not Tomorrow, Inc-Project Babies, Meadowood Neighborhood Center and Neighborhood Connectors.

Access Community Health Centers is closely associated with this partnership and they provide technical

and medical information assistance. After the release of The Race to Equity Report (2013), there was a

compelling need for community residents, city agencies and local non-profits to come together to seek

solutions to the disparities outlined in the Report. MHP seeks to fill this void by connecting, engaging,

sharing, learning and serving local residents in the Meadowood community.


Physicians and health care workers partner with MPH to provide health care support services. MPH has

conducted well-child clinics provided diabetes education for community members.


Community Health Worker

Have a health concern?  Wondering what community resources are available to help you?  Sheray Wallace, the first community health worker in Dane County, is here to help!  All services provided by the community health worker are free and open to all community members.  MPH is now collaborating with Access Health Clinic and WI Kids Forward to provide even better support for all community members!

Newly pregnant and looking for prenatal care?  Meadowood Health Partnership is partnering with Joining Forces for Families to bring prenatal care and education to the Meadowood Neighborhood.
